Forms for Privacy Complaints or Requests for Review for use in British Columbia

For Public bodies in the Province of British Columbia - The Freedom of Information and Protection of Privacy Act (FOIPPA) provides the right to request a review of a public body's decision respecting your request for information or possible misuse of personal information.

The following links will take you to an Adobe Acrobat form for the purpose that the link displays and the form can be filled out online. Unfortunately, once completed, the form cannot be saved to your computer but you can print it off using your browser's or Abode Reader's print button. It is useful to complete the form online and when you are satisfied, print off two copies - one for you to keep and one to send to the Public Body (Step 1) and the Office of the Information and Privacy Commissioner (OIPC) (Step 2). You can also just print the form and fill it out manually. After you are finished with the form and you wish to return to this or a previous page, just use your browser's back button after you have closed out of the Adobe Reader, if it was launched on your system.
